Client Records and Complaints

Use the instructions below to request your records or contact the appropriate Texas agency about a privacy or licensing concern.

Request your client records

Email [email protected] and include:

  1. Your full name.
  2. The approximate date or date range of services.
  3. A description of the records you are requesting.
  4. Your preferred delivery method.

Identity verification may be required before records are released. Please do not send sensitive personal information, identification documents, or medical details in your first email. You will receive follow-up instructions if additional information is needed.

Privacy and licensing complaints

Texas law provides certain rights regarding privacy of and access to health care records (Texas Health & Safety Code, Chapter 181, Medical Records Privacy). To file a consumer complaint alleging a violation of your privacy rights regarding protected health information, contact the Texas Office of the Attorney General, Consumer Protection Division.
